Does a Diaper Bag Count as a Carry-On?
The short answer is 'check your airline' — so here is what each of the big four actually publishes, and where the published record runs out.

The short answer
It depends on the airline, and the published policies are less generous and less clear than the internet suggests. Plan on your diaper bag being your personal item unless your airline states otherwise in writing. Strollers and car seats are separate free items on the carriers we checked; a diaper bag usually is not.
This question gets answered confidently all over the internet and the published record does not support most of those answers. Below is what each of the four largest US carriers actually states on its own pages, quoted with the date we read it — and, where a page could not be read, we say so rather than filling the gap.

The gap in the published record
We could not read United’s family-travel page in full: it renders its policy detail with client-side JavaScript and returns an empty shell to a plain fetch. We attempted it directly and via a site-scoped search on 2026-09-27. United’s carry-on dimension (22 x 14 x 9 in) and the statement that FAA-approved child restraint systems may be brought in addition to a carry-on and personal item were readable and are cited above. Its specific diaper bag allowance was not, and we are not going to invent one.
Similarly, Delta’s infant travel page does not state a diaper bag allowance or a stroller check policy — it points to a separate infant and children’s items page. We are reporting the absence rather than reasoning from another airline’s rules.
The practical plan, whatever your airline says
Treat the diaper bag as your personal item. Then it fits whatever the rule turns out to be, and you are never the family being asked to consolidate at the gate.
Choose a bag under about 9 in deep. That is the dimension that fails an under-seat check. Of the bags in our diaper bag roundup, the Ruvalino at 7.8 in and the Skip Hop Forma at 8 in are the two most likely to pass.
Use the stroller and car seat allowance instead. On the carriers that publish it, those are genuinely free and separate — Southwest gives one of each per ticketed customer. That is where the free capacity actually is. See gate-checking a stroller.
Keep liquids in one accessible pouch. Formula and breast milk get screened separately, and a pouch you can hand over in one motion turns that into a fifteen-second event.

Questions parents actually ask
Is a diaper bag free on a plane?
It varies by airline and the published record is thinner than you would expect. United publishes that on most flights you get one carry-on and one personal item, plus additional items including FAA-approved child restraint systems. American publishes a free checked bag allowance for an infant, up to 25 lb and 22 x 14 x 9 in. Neither statement is the same as 'a diaper bag is always free on top'. Check with your carrier before you fly.
Can a lap infant have their own bag?
On Southwest, no: it publishes plainly that a lap child may not check or carry on any baggage. That is the clearest published position of the four carriers we looked at, and it is worth knowing before you plan around an extra allowance.
What size does a diaper bag need to be?
If it is your personal item, it needs to fit under the seat, and depth is the dimension that fails. United publishes a carry-on maximum of 22 x 14 x 9 in; personal item limits are tighter. A diaper bag 12 in deep packed full will not go under many seats, whatever its volume.
Do strollers and car seats count against my bags?
No, on the carriers we checked. Southwest publishes one stroller and one car seat free per ticketed customer, checkable at the curb, counter or gate. American publishes that a fully collapsible stroller under 20 lb can be carried on if it fits an overhead bin, and gate-checked free when traveling with a child.
What about formula and breast milk?
Those are screened separately at security in the US and can exceed the normal liquid limits, but they have to come out of the bag for screening. Declare them rather than hoping — and keep them in one accessible pouch so it takes seconds.
